How Wholistic Care Helps OFN's Young Adults Reach Their Full Potential

In 2016 the Canadian Human Rights Tribunal ("CHRT") found that Canada discriminated against First Nation children, young adults, and communities through, in part, providing inequitable and insufficient funding for child and family services (“CFS”). This included not providing support to young adults who were previously in care after they aged out. So, the CHRT made it clear that Canada needed to develop a system to ensure that support is provided to young adults who were previously in care and make sure they have the opportunity to reach their full potential.

 

To make this happen, in 2022 Indigenous Services Canada ("ISC") developed the Post-Majority Support Services ("PMSS") system specifically for those that were in care at some time in their childhood and are currently between the ages of 19 and 26. Further, because ISC recognized that on-reserve CFS agencies such as OCFS are already overworked and overstretched, they've allowed PMSS support to be provided to eligible young adults by “Authorized Service Providers” such as Wholistic Care.


As discussed below, each eligible young adult that signs up to receive PMSS support from Wholistic Care will work one-on-one with a Case Manager. This Case Manager will assess the young adults current needs and their long-term goals and develop a Service Delivery Plan to try to turn their goals into a reality (or make significant progress on them) before the age of 26.


Based on the Service Delivery Plan created and what the Case Manager believes is in the long-term best interests of the young adult, Wholistic Care will provide both financial and non-financial support to ensure that the young adult's transition into adulthood goes as well as possible.


While the PMSS system can seem complicated at first, the bottom line is that moving forward the transition into adulthood for an individual that’s been in care at some point during their childhood will be much less overwhelming and much less distressing because now there will be a financial and non-financial support system in place to provide as much (or as little) support as needed from the ages of 19 to 26.

 

In the end, the mission of both Wholistic Care and ISC’s newly developed PMSS system is simple: to ensure that all young adults who were previously in care have access to the services, support, and care that they need to realize their full potential and be self-sufficient by the time they turn 26.

Financial Support and Housing

Wholistic Care can and will help offset basic living expenses - such as rent, utilities, transportation, and groceries - for those in need until the age of 26. The level of support that can be provided will be based on a Case Manager’s detailed assessment of each young adult’s current income, current expenditures, and the funds they will need to achieve their long-term goals. However, in order to receive ongoing financial support from Wholistic Care there will be some strings attached to ensure that all young adults are using the financial support provided to truly meet their long-term goals.

Education and Professional Development

Wholistic Care understands that many young adults have dreams of pursuing higher education or some form of professional development but are discouraged by the significant costs involved. The Case Manager assigned to each young adult will work with them to create a concrete plan and Wholistic Care will help fund education and professional development costs such as hiring mentors, purchasing books, setting up meetings with career counselors, and even funding one-off purchases such as a laptop or work-related tools.

Physical, Mental, and Social Well-Being

The transition from childhood to adulthood comes with many challenges and this can sometimes lead to ignoring one’s physical, mental, and social health. But no matter what one’s long-term goals are, they can’t be accomplished without a strong physical, mental, and social foundation. So, Wholistic Care’s Case Managers will try their hardest to ensure all young adults have access to the supports they need to thrive – this can include Wholistic Care helping to pay for everything from dental services to mental health counseling to parenting workshops to cooking classes to gym memberships.

Land, Culture, Language, and Community

Core to Wholistic Care’s mission is ensuring that young adults who were previously in care don’t see their connection to their community and culture fade away. As a result, there will always be funding available (if needed) for young adults to participate in language classes, purchase regalia, travel back to OFN for events, etc. Further, Wholistic Care has a dedicated Cultural Coordinator who will conduct regular group classes, land-based activities, and be available for one-on-one mentorship.

How Wholistic Care Works…

ISC’s PMSS system only allows PMSS support to be provided to eligible young adults (those that were previously in care but are now between the ages of 19 and 26) through either a First Nation or an Authorized Service Provider like Wholistic Care.


For those that are eligible, signing up to receive PMSS support through Wholistic Care is (of course) completely free and completely voluntary. Once someone has signed up, they will become a Wholistic Care “Client” and will have a dedicated Case Manager that until the age of 26 will be tasked with trying to help them reach their long-term goals in whatever ways are possible.


ISC’s PMSS system has some limitations, and thus Wholistic Care is limited in both the type and the level of support that it can provide. However, each Client’s Case Manager will try their hardest to craft a Service Delivery Plan based on the Client’s current situation and their dreams for the future and provide the maximum amount of support possible.


For those eligible to receive PMSS support, here are the four basic steps involved…

01- Sign-Up for Wholistic Care

First, an eligible young adult should contact Wholistic Care and a Case Manager will be more than happy to talk to them. The Case Manager will explain in more detail what PMSS support involves and how Wholistic Care can help them. If they’re still interested and are excited to start getting some support, then they can sign-up and become a Client of Wholistic Care. This is (of course) completely free and the Client can end their enrollment in Wholistic Care’s PMSS Program at any time and for any reason.

02- Complete Orientation Materials

Once signed-up, the Client will meet with the Case Manager to complete the “Orientation Materials”. These Orientation Materials will help the Case Manager better understand the Client’s current financial situation, what specific aspects of their life they’d like to improve, and what their long-term goals are. This will then enable the Client’s Case Manager to figure out how Wholistic Care can best support the Client – both financially and non-financially – and ensure that they reach their long-term goals and are self-sufficient by the age of 26.

03- Review Service Delivery Plan and Support

Based on the completed Orientation Materials and the Case Manager’s assessment of the Client, the Case Manager will create a Service Delivery Plan. This is a preliminary overview of how Wholistic Care plans to help the Client reach their goals and be self-sufficient by the age of 26. The Case Manager will then review what their overall plan is with the Client and discuss the amount of financial support that Wholistic Care can provide on an ongoing basis and what obligations the Client will have in order to continue receiving this support each month.

04- Work Toward Long-Term Goals

After this, until the Client turns 26, the Case Manager will act as both a mentor and counsellor to the Client: always being available if the Client needs help and ensuring that the Client is making progress toward their long-term goals. To this end, the Case Manager will reach out once a week (if not more) to make sure that everything is going well with the Client. Further, every three months the Client will need to sit down with the Case Manager to discuss their progress and what, if any, changes need to be made to the financial and non-financial support they’re being provided in order to ensure that by 26 they're self-sufficient, independent, and on a stable pathway forward.
