How Wholistic Care Helps OFN's Young Adults Reach Their Full Potential
How Wholistic Care Works…
The Post-Majority Support Services ("PMSS") system only allows PMSS support to be provided to eligible young adults (those that were previously in care but are now between the ages of 19 and 26) through either a First Nation or an Authorized Service Provider like Wholistic Care.
For those that are eligible, signing up to receive PMSS support through Wholistic Care is (of course) completely free and completely voluntary. Once someone has signed up, they will become a Wholistic Care “Client” and will have a dedicated Case Manager that until the age of 26 will be tasked with trying to help them reach their long-term goals in whatever ways are possible.
The PMSS system has some limitations, and thus Wholistic Care is limited in both the type and the level of support that it can provide. However, each Client’s Case Manager will try their hardest to craft a Service Delivery Plan based on the Client’s current situation and their dreams for the future and provide the maximum amount of support possible.
For those eligible to receive PMSS support, here are the four basic steps involved…
01- Sign-Up for Wholistic Care
First, an eligible young adult should contact Wholistic Care and a Case Manager will be more than happy to talk to them. The Case Manager will explain in more detail what PMSS support involves and how Wholistic Care can help them. If they’re still interested and are excited to start getting some support, then they can sign-up and become a Client of Wholistic Care. This is (of course) completely free and the Client can end their enrollment in Wholistic Care’s PMSS Program at any time and for any reason.
02- Complete Orientation Materials
Once signed-up, the Client will meet with the Case Manager to complete the “Orientation Materials”. These Orientation Materials will help the Case Manager better understand the Client’s current financial situation, what specific aspects of their life they’d like to improve, and what their long-term goals are. This will then enable the Client’s Case Manager to figure out how Wholistic Care can best support the Client – both financially and non-financially – and ensure that they reach their long-term goals and are self-sufficient by the age of 26.
03- Review Service Delivery Plan and Support
Based on the completed Orientation Materials and the Case Manager’s assessment of the Client, the Case Manager will create a Service Delivery Plan. This is a preliminary overview of how Wholistic Care plans to help the Client reach their goals and be self-sufficient by the age of 26. The Case Manager will then review what their overall plan is with the Client and discuss the amount of financial support that Wholistic Care can provide on an ongoing basis and what obligations the Client will have in order to continue receiving this support each month.
04- Work Toward Long-Term Goals
After this, until the Client turns 26, the Case Manager will act as both a mentor and counsellor to the Client: always being available if the Client needs help and ensuring that the Client is making progress toward their long-term goals. To this end, the Case Manager will reach out once a week (if not more) to make sure that everything is going well with the Client. Further, every three months the Client will need to sit down with the Case Manager to discuss their progress and what, if any, changes need to be made to the financial and non-financial support they’re being provided in order to ensure that by 26 they're self-sufficient, independent, and on a stable pathway forward.
